Cosopt (Dorzolamide and Timolol) is a prescription eye drop that lowers high pressure in the eye associated with open-angle glaucoma and ocular hypertension. By combining two active ingredients, it effectively reduces fluid production in the eye to help prevent vision loss and protect the optic nerve.

What is Cosopt?

Cosopt is a prescription eye drop medication containing dorzolamide and timolol, used to treat high pressure within the eye (intraocular pressure) in patients with open-angle glaucoma or ocular hypertension. High pressure can damage the optic nerve at the back of the eye, leading to gradual vision loss. Cosopt is a combination therapy designed to lower this internal pressure and help protect your sight. It is typically prescribed when a single medication is not sufficient to achieve the target eye pressure set by your doctor.

How does Cosopt work?

Cosopt works by combining two different active ingredients, dorzolamide and timolol, to decrease the amount of fluid (aqueous humor) produced inside the eye, which in turn lowers intraocular pressure. Dorzolamide belongs to a class of drugs called carbonic anhydrase inhibitors. It works by blocking an enzyme that is critical for fluid production in the eye. Timolol is a beta-blocker that also reduces the production of aqueous humor through a different mechanism. The combined action of these two ingredients results in a more significant pressure reduction than either drug could achieve on its own.

Who is Cosopt for?

Cosopt is for adults diagnosed with open-angle glaucoma or ocular hypertension who a doctor has determined require a combination therapy to effectively lower their intraocular pressure. It is specifically intended for patients who have not responded adequately to treatment with a single beta-blocker medication or for whom a combination of two drugs is considered a necessary first-line treatment. This medication is not generally recommended for children, and its use should be based on a thorough evaluation by an ophthalmologist to ensure it is the appropriate choice for your specific condition.

Dosage and administration

The standard dosage for Cosopt is one drop in the affected eye or eyes twice daily, administered approximately 12 hours apart. To apply the drops correctly, first wash your hands. Tilt your head back, look up, and gently pull down the lower eyelid to create a small pocket. Squeeze the bottle to release a single drop into this pocket without letting the tip touch your eye. Close your eye for 1-2 minutes and press gently on the corner of your eye near the nose to prevent the medication from draining into your tear duct. If you use other eye medications, wait at least 10 minutes between applications. Always follow your doctor’s specific instructions.

Side effects

Common side effects of Cosopt include temporary stinging or burning in the eye upon application, a bitter or unusual taste in the mouth, and short-term blurred vision. Other localized effects may include eye redness, itching, or the feeling of something being in the eye. Because Cosopt contains a beta-blocker (timolol) that can be absorbed into the bloodstream, it can potentially cause systemic side effects, though this is less common. These can include a slowed heart rate, fatigue, or shortness of breath. You must inform your doctor of your full medical history, especially of any heart or breathing conditions, before starting treatment. If any side effects are severe or persistent, contact your doctor for advice.

Like all medications, Cosopt can cause side effects. Common side effects often experienced in and around the eye include:

  • Stinging or burning sensation upon application
  • Eye redness, itching, or dryness
  • Blurred vision immediately after use
  • A feeling as if something is in the eye
  • Bitter or unusual taste in the mouth

While less common, some side effects can be serious due to the medication being absorbed into the bloodstream. Contact your doctor immediately if you experience:

  • Shortness of breath, wheezing, or asthma symptoms
  • Slow or irregular heartbeat
  • Severe dizziness or fainting
  • Muscle weakness
  • Swelling in the hands or feet

Cosopt is indicated for the reduction of elevated intraocular pressure (IOP) in patients with:

  • Open-angle glaucoma
  • Ocular hypertension

It is used when monotherapy with a beta-blocker eye drop is not sufficient to control eye pressure.
