Orencia (generic name: abatacept) can be prescribed to treat moderate to severe immune-driven joint and autoimmune conditions. It belongs to the pharmacological class of selective T-cell costimulation modulators, which act by disrupting part of the immune system that contributes to chronic inflammation in diseases such as rheumatoid arthritis(RA), psoriatic arthritis (PsA) and polyarticular juvenile idiopathic arthritis (pJIA).
Abatacept is a biologic medicine that works by binding to the immune-cell proteins CD80 and CD86, thereby preventing T-cells from receiving a key activation signal. This mechanism helps reduce the ongoing immune attack on joints and other tissues.
By reducing inflammation, Orencia helps ease symptoms such as joint swelling, persistent pain, and functional limitations. In many patients with moderate to severe disease who have not gained sufficient benefit from other treatments, this therapy offers an additional option to help stabilise their condition and support daily activities. It’s intended for longer-term management, rather than short-term relief only.
Dosage
It is important to follow the dosage instructions printed on the label of the box. Check with a doctor or pharmacist if there are any doubts regarding the proper dosage and treatment regimen.
Orencia is administered either as an intravenous infusion or a subcutaneous injection, depending on patient age and formulation. For adult rheumatoid arthritis, dosing typically begins with a loading intravenous infusion followed by regular maintenance dosing, or direct weekly subcutaneous injections of 125 mg.
-
The intravenous form is given as a dose depending on body weight, then repeated at specified intervals, transitioning to the subcutaneous form if needed.
-
For subcutaneous injection, the dose is usually once weekly; patients should use the same day each week for consistency.
-
If a dose is missed, it should be taken as soon as possible or at the next scheduled time. Do not take two doses at once to make up for the missed one.
-
After symptoms are well controlled, the doctor may adjust the regimen to the lowest effective dose and reassess periodically.

Typical symptoms include:
-
joint swelling (often in hands, wrists, or feet)
-
persistent pain and tenderness in the joints
-
morning stiffness lasting more than 30 minutes

- Psoriatic arthritis (PsA)
Typical symptoms include:
-
joint pain and swelling, sometimes in fingers or toes (sausage-like appearance)

skin changes (psoriasis) in conjunction with joint symptoms
In PsA, the immune system drives inflammation in skin and joints. Orencia helps reduce both joint and skin manifestations.
- Polyarticular juvenile idiopathic arthritis (pJIA)
Typical symptoms include:
-
young children or adolescents aged 2 years and older experiencing joint pain and swelling in five or more joints
-
stiffness, possibly affecting mobility or school activities

signs of inflammation over time
Orencia is used in children aged 2 years and older for this condition, either alone or with methotrexate.
In each condition, the aim is to reduce disease activity, prevent joint damage, and support function and quality of life over the long term. The decision to use Orencia should be guided by a specialist who assesses disease severity, prior treatments, and overall health status.
