Nexletol / Nilemdo (generic name: bempedoic acid) is a prescription medication used to help lower LDL-C cholesterol (“bad” cholesterol) levels in adults with specific heart or cholesterol conditions. Bempedoic acid works by inhibiting an enzyme called ATP-citrate lyase, which plays an essential role in the liver’s production of cholesterol. By blocking this enzyme, Nexletol reduces the amount of cholesterol produced by the liver, thus lowering the overall level of LDL cholesterol in the bloodstream. Lowering LDL cholesterol is crucial for reducing the risk of heart attack, stroke, and other cardiovascular events in individuals who have difficulty controlling their cholesterol levels through diet and statins alone. Nilemdo is FDA-approved for individuals with heterozygous familial hypercholesterolemia (HeFH) or established atherosclerotic cardiovascular disease (ASCVD) for whom diet and maximally tolerated statins are insufficient to lower their cholesterol levels. Bempedoic acid offers an alternative treatment for patients who may not tolerate high doses of statins due to muscle pain or other side effects. Other drugs in the same class, such as Nexletol or Nexlizet  (which combines bempedoic acid with ezetimibe), are also commonly prescribed to manage cholesterol levels. The prescribing physician determines the choice between these options based on the patient’s specific needs.

Caution

Nelextol has not been fully tested on pregnant women. Pregnant women or those planning to become pregnant should consult their healthcare provider before starting this medication.

Dosage

Follow the healthcare provider’s instructions on dosing. Patients should not stop taking Nexletol without consulting their doctor, even if their cholesterol levels improve. The standard dosage of Nexletol is one 180 mg tablet taken once daily, with or without food. It should be taken at the same time each day. If a dose is missed, it can be taken on the due day as soon as realized, but if not, do not double-dose the next day to make up for a missed dose.

Storage

Warnings:

Nexletol may increase the risk of tendon rupture, especially in patients over 60 years of age, on corticosteroid therapy, or with pre-existing kidney failure. Patients should immediately report any sudden pain, swelling, or difficulty moving a tendon to their healthcare provider.

Any signs of jaundice (yellow skin or eyes) or severe fatigue should be immediately reported to a healthcare provider.

  • Increased uric acid levels, which may lead to gout
