Intuniv XR (generic name: guanfacine) is a non-stimulant extended-release tablet used to treat Attention-deficit/Hyperactivity Disorder (ADHD) in children and adolescents aged 6 to 17 years. It belongs to the class of medicines called alpha-2A adrenergic agonists. Guanfacine works by strengthening regulation of attention and impulse control through its action on the prefrontal cortex. It can be taken alone or alongside stimulant medicines, providing symptom relief without the abuse potential of stimulants. Treatment is once daily, designed for steady coverage throughout the day. Usual dosage

It is important to follow the dosage instructions printed on the label of the box. The usual starting dose is 1 mg once daily, taken in the morning or evening. The dose may be increased by 1 mg per week depending on response, typically reaching 3–4 mg daily. The maximum dose is 4 mg/day in children 6–12 years and 7 mg/day in adolescents 13–17 years. Dosing may also be weight-based (0.05–0.12 mg/kg/day). Tablets should be swallowed whole. Do not crush or chew. If a dose is missed, skip it and continue with the next scheduled dose. Do not double dose to make up for a missed dose. Caution: do not discontinue abruptly, as this may cause rebound high blood pressure; taper gradually under medical advice.

Standard storage

Store Intuniv XR tablets at 20-25 °C (68-77 °F). Protect from moisture and light. Store in the original container with the cap closed. Keep out of reach of children. Do not use beyond the expiry date, which refers to the last day of the month printed on the package.

Caution

If a sudden allergic reaction (anaphylaxis) occurs shortly after taking Intuniv XR, with symptoms like swelling of the face, tongue, or throat making it difficult to breathe or swallow, or there is wheezing, hives, rash, blistering, or peeling of the skin, call a doctor or 911 right away, or go to an emergency room immediately.

Pregnant women should not be using Intuniv XR unless this has been discussed with a healthcare provider, as it might harm the unborn baby or pose a risk of miscarriage. For this reason, women planning to become pregnant should use appropriate birth control measures during treatment and when tapering off. Intuniv XR may pass into breast milk, and mothers should consult with a doctor if already feeding or planning to feed an infant.

Most people who use Intuniv XR do not experience any adverse side effects. Doctors prescribe this medication because they assess the benefits of such treatment outweigh any likely unwanted effects.

Some of the side effects that have been reported include:

  • Dizziness or lightheadedness
  • Dry mouth
  • Constipation
  • Headache

Guanfacine may also be used  off-label for related conditions such as tics, aggression, anxiety, and sleep disturbances, but these uses require specialist guidance.

  • Some improvement may be seen within 1 to 2 weeks, but full benefits usually occur after several weeks of regular daily dosing. Consistency is important for maximum effect.
