Xofluza (generic name: baloxavir marboxil) is an antiviral medication designed to treat and prevent infection by influenza A and B viruses in individuals aged 12 and older. It is particularly effective for acute uncomplicated influenza, which occurs when symptoms have been present for no more than 48 hours. Xofluza offers a single-dose treatment regimen, simplifying patient compliance. Its mechanism of action involves inhibiting the cap-dependent endonuclease enzyme, a key component of the influenza virus’s replication process. This disruption reduces the virus’s ability to multiply within the body, aiding symptom resolution and limiting further spread. As a member of the polymerase acidic (PA) endonuclease inhibitors class of medications, Xofluza is a targeted therapy that distinguishes itself from other antiviral drugs. It is effective both as a treatment for active infections and as a post-exposure prophylactic to prevent illness in individuals exposed to the flu. However, Xofluza is not a replacement for influenza vaccination, which remains the primary method of flu prevention. For more information about influenza and its treatment options, visit the CDC Influenza Treatment page.

Dosage

Doctors will set the appropriate dosage. For patients weighing less than 80 kg, the usual dose is 40 mg as a single oral dose. Patients weighing 80 kg or more should be given 80 mg as a single oral dose. Xofluza should be taken as soon as possible after the onset of symptoms or exposure, preferably within 48 hours. It can be taken with or without food. However, avoid dairy products, calcium-fortified beverages, or antacids containing polyvalent cations, as these may reduce the drug’s absorption. If a dose is missed, contact a healthcare provider for advice, as Xofluza is generally a one-time, single-dose treatment.

Storage

Store Xofluza at room temperature at 20°-25°C (68°-77°F). Keep the medication in its original packaging to protect it from moisture and light. Avoid storing it in the bathroom or exposing it to excessive heat.

Common Questions About Xofluza

How does Xofluza treat influenza?Xofluza treats influenza by targeting a key step in the virus’s replication process. It inhibits the cap-dependent endonuclease enzyme, a vital component for the transcription of viral RNA. By halting this process, Xofluza prevents the virus from multiplying within the body. This action helps reduce the duration and severity of symptoms. Unlike traditional antivirals requiring multiple doses, Xofluza offers a single-dose regimen, simplifying treatment. It is most effective when taken within 48 hours of symptom onset. For prevention, it can be used shortly after exposure to the influenza virus to stop the illness from developing.
Can Xofluza replace the flu vaccine?
Xofluza cannot replace the flu vaccine. The flu vaccine is designed to provide immunity against specific influenza strains and is the primary method of preventing influenza each season. Xofluza, on the other hand, is an antiviral medication used to treat active flu infections or prevent flu in individuals recently exposed to the virus. Vaccination provides long-term protection, whereas Xofluza addresses immediate cases of illness or exposure. Health experts strongly recommend annual flu vaccinations for individuals six months and older to reduce the risk of flu-related complications and hospitalizations.
Why is early treatment with Xofluza important?
Xofluza is most effective when administered within 48 hours of symptom onset. During this window, the influenza virus is actively replicating, and the drug can significantly limit its spread in the body. Delaying treatment reduces its effectiveness, as the virus may already have caused substantial damage. Early treatment not only shortens the duration of symptoms but also reduces the risk of complications, such as bacterial infections or hospitalization, especially in high-risk individuals like the elderly or those with chronic health conditions.

Read the information leaflet that comes with the medication.

If a sudden allergic reaction (anaphylaxis) occurs after taking Xofluza, with symptoms like swelling of the face, tongue, or throat making it difficult to breathe or swallow, or there is wheezing, hives, rash, blistering, or peeling of the skin, call a doctor or 911 right away, or go to an emergency room immediately.

Most people who use Xofluza do not experience any adverse side effects. Doctors prescribe this medication because they assess the benefits of such treatment outweigh any likely unwanted effects.

Some of the side effects that have been reported include:

  • Diarrhea
  • Nausea
  • Bronchitis
  • Sinusitis
  • Headache

These side effects are generally mild and resolve without intervention.

Not all side effects are listed here. If these or other unlisted symptoms persist or worsen, consult a healthcare provider or pharmacist.

Xofluza is approved for individuals aged 12 and older. Xofluza is primarily used to treat and prevent acute uncomplicated influenza. Influenza symptoms include fever, chills, cough, sore throat, runny or stuffy nose, muscle aches, headaches, and fatigue. Early treatment with Xofluza can reduce the severity and duration of these symptoms.

In addition to its primary indications, Xofluza may be used off-label for post-exposure prophylaxis in high-risk populations to prevent the spread of influenza in close-contact settings.

Its use in younger populations and during pregnancy or breastfeeding has not been fully evaluated. It is not indicated for severe or complicated influenza cases requiring hospitalization.

Key Benefits of Xofluza

  1. Single-Dose Regimen: Simplifies treatment compared to multi-dose antiviral therapies.
  2. Dual Use: Treats active flu infections and prevents flu in exposed individuals.
  3. Rapid Action: Reduces symptom duration when taken promptly.

  • Who should take Xofluza?
Xofluza is recommended for individuals aged 12 years and older who are experiencing acute, uncomplicated influenza. It is also suitable for people who have been exposed to someone with the flu and want to prevent illness. However, it is not recommended for pregnant or breastfeeding women unless advised by a healthcare provider. Additionally, Xofluza is not approved for children under 12 or for treating severe cases of influenza requiring hospitalization. Consulting a healthcare provider can help determine if Xofluza is appropriate based on individual health conditions and circumstances.
  • What symptoms does Xofluza relieve?
Xofluza is designed to treat the symptoms of acute, uncomplicated influenza, including fever, chills, cough, sore throat, runny or stuffy nose, muscle or body aches, headaches, and fatigue. It reduces the duration and severity of these symptoms when taken within 48 hours of their onset. While Xofluza is not a cure for influenza, it helps the body recover faster by limiting the virus’s ability to multiply. It does not address bacterial infections or other complications that may arise from the flu, so additional treatments might be required in such cases.
  • How does Xofluza help prevent the flu?
Xofluza can prevent influenza when taken after exposure to someone with the virus. This use is known as post-exposure prophylaxis. By inhibiting the replication of the influenza virus, Xofluza stops the infection from developing in individuals who have been exposed but are not yet symptomatic. It is particularly beneficial in close-contact settings, such as households or schools, where the flu spreads easily. However, it should not replace other preventive measures, such as vaccination, hand hygiene, and avoiding contact with infected individuals.
