Opsumit (generic name: macitentan) is prescribed for the treatment of a specific form of pulmonary arterial hypertension –  PAH WHO Group I.

WHO Group 1 PAH is a rare subset of pulmonary hypertension, a serious lung disease that primarily affects the small pulmonary arterioles – the air sacs that absorb oxygen and release carbon dioxide. As the condition develops, PAH causes progressive blood flow restriction and forces the heart to work harder, eventually leading to heart failure.

This class of orally administered dual endothelin receptor antagonist medication works by blocking the vasoconstrictor that is upregulated in pulmonary arterial hypertension (PAH). Opsumit promotes vasodilation, reduces pulmonary vascular resistance, and helps prevent vascular remodeling in pulmonary arteries. It belongs to a pharmacologic class designed to reduce disease progression and lower the rate of pulmonary arterial hypertension‑related hospitalizations.

Caution

Opsumit can cause serious birth defects if taken during pregnancy. Females should not be pregnant when they start taking Opsumit or become pregnant during treatment with this medication. If you think you may have become pregnant while being treated with Opsumit, talk to a doctor right away.

Dosage

Follow the instructions on the label of the Opsumit box. If you have any questions, consult with a doctor or pharmacist. 

The recommended dosage of Opsumit is one 10 mg tablet taken once daily, at the same time each day, with or without food. Tablets should be swallowed whole and not split, crushed, or chewed. If a dose is missed and it’s still the same day, take the missed dose as soon as remembered. If it’s the next day, skip the missed dose. Do not double the dose to make up for a missed one.

Don’t stop taking the medicine without consulting a doctor.

Storage

Opsumit tablets should be stored at room temperature, ideally between 20 °-25 °C (68 –77 °F), and kept in the original container away from moisture and direct sunlight. If in any doubt, consult our blog on safe medicine management.

Patients should seek immediate medical attention if they experience signs of liver injury (jaundice, yellowing of the skin or whites of the eyes,) or pregnancy. If a sudden allergic reaction (anaphylaxis) occurs shortly after taking a dose of Opsumit, with symptoms like swelling of the face, tongue, or throat making it difficult to breathe or swallow, or there is wheezing, hives, rash, blistering, or peeling of the skin, call a doctor or 911 right away, or go to an emergency room immediately. 

Most people who use Opsumit do not experience any adverse side effects. Doctors prescribe this medication because they assess the benefits of such treatment outweigh any likely unwanted effects.

Some of the side effects that have been reported include headache, upper respiratory infections, nasopharyngitis, bronchitis, flu-like symptoms, and urinary tract infections.

Opsumit is FDA-approved for pulmonary arterial hypertension (PAH, WHO Group I). PAH is a progressive condition marked by elevated pulmonary arterial pressure and resistance, leading to strain on the right side of the heart.

Symptoms of PAH include persistent shortness of breath, fatigue, dizziness or fainting, chest discomfort, swelling in lower limbs, and rapid heartbeat. These symptoms often worsen with physical exertion.

Opsumit can also be prescribed off-label for pulmonary fibrosis when it is associated with pulmonary hypertension.

  • While some effects may appear after weeks, clinical benefits, like improved exercise capacity and reduced hospitalization, are typically observed over several months, as demonstrated by the SERAPHIN trial.
