Accupril (generic name: quinapril) is an ACE inhibitor. It helps treat hypertension (high blood pressure) and is also used with other medicines for heart failure.

Quinapril lowers blood pressure by blocking the body’s angiotensin-converting enzyme, which reduces angiotensin II, a chemical that tightens blood vessels. When vessels relax, the heart pumps against less resistance, blood pressure falls, and long-term risks like stroke and heart attack are reduced. In heart failure, lowering the pressure the heart works against can help symptoms and reduce hospital visits. More information about the functions of ACE inhibitors like quinapril can be found on its MedLinePlus page.

Dosage

Read the information on the patient leaflet inside the box. Dosing is individualized. For patients not on a diuretic, typical labeling cites an initial 10–20 mg once daily, with adjustment by blood-pressure response. Many patients need 20–80 mg per day, as a single daily dose or divided twice daily. In heart failure, care teams usually start low and increase slowly while watching blood pressure, kidney function, and potassium.

Tablets should be swallowed with water and may be taken with or without food. Dosing times should be consistent each day.

Accupril is a daily maintenance medicine. Treatment should continue even when symptoms are absent, because high blood pressure is often silent. It is not for fast relief of chest pain, breathlessness, or other urgent symptoms; those situations require a separate rescue plan from a clinician.

If a dose is missed and the next dose is not soon, the missed dose may be taken when remembered. If the next dose is due within 3-4 hours, skip the missed dose and return to the regular schedule. Do not take two doses at once.

Storage

Store tablets tightly closed at room temperature, away from moisture and heat. Keep out of the reach of children. Do not use after the printed expiry date. No refrigeration is required.

This text is for informational purposes only. Please consult a doctor or pharmacist before using any medication.

The information leaflet that comes with the medication includes full information about the correct usage, storage and possible side effects. Be sure to read and understand these, and if there are any questions, discuss these with a healthcare provider or pharmacist before starting treatment.

Caution:

If a sudden allergic reaction (anaphylaxis) occurs shortly after taking Accupril, with symptoms like swelling of the face, tongue, or throat making it difficult to breathe or swallow, or there is wheezing, hives, rash, blistering, or peeling of the skin, call a doctor or 911 right away, or go to an emergency room immediately.

Pregnant women should not be using Accupril unless this has been discussed with a healthcare provider, as it might harm the unborn baby or pose a risk of miscarriage. For this reason, women planning to become pregnant should use appropriate birth control measures during the treatment and one month after taking the last dose of Accupril.

Most people who use Accupril do not experience any adverse side effects. Doctors prescribe this medication because they assess the benefits of such treatment outweigh any likely unwanted effects.

Some of the side effects that have been reported include:

  • headache
  • cough
  • dizziness
  • tiredness
  • nausea, vomiting, or stomach pain

Serious side effects can include swelling of the face, lips, tongue, or throat (angioedema), high potassium, kidney or liver problems, or fainting. Get medical help right away.

Not all side effects are listed here. If these or other unlisted symptoms persist or worsen, consult a healthcare provider or pharmacist.

Quinapril is prescribed for:

Hypertension (high blood pressure): A long-term condition where blood presses too hard on artery walls. Most people have no clear symptoms. When present, signs can include morning headaches, dizziness, or vision changes—usually with very high readings. Over time, uncontrolled pressure damages the brain, heart, kidneys, and eyes. Quinapril relaxes blood vessels, lowers pressure, and helps reduce these risks when taken regularly with monitoring.

Adjunct in heart failure: A chronic condition where the heart cannot pump enough blood to meet the body’s needs. Common symptoms include shortness of breath, swelling in the legs or ankles, fatigue, and reduced exercise capacity. Quinapril lowers vascular resistance and helps the heart work more efficiently as part of a broader treatment plan that may include diuretics and other medicines. Regular follow-up helps track benefits and safety. Kaiser Permanente

Common clinical practice (selected): In patients with diabetes or chronic kidney disease and albumin in the urine, ACE inhibitors are often used to protect kidney function; decisions are individualized and monitored by clinicians. NCBI

  • Accupril is often combined with other antihypertensives to reach target blood pressure. Thiazide diuretics are a common partner. Certain combinations need extra care: potassium-sparing diuretics, potassium supplements, or salt substitutes can raise potassium; NSAIDs may blunt the effect and affect kidneys; aliskiren is generally avoided in diabetes. Clinicians review all medicines, check kidney function and potassium, and watch for low blood pressure, especially at the start or after dose changes.
