ProAmatine / Midodrine (midodrine hydrochloride) can be prescribed to treat adults diagnosed with severe orthostatic hypotension, which is a form of chronic low blood pressure. The primary symptoms are dizziness when standing, sometimes leading to fainting. This is typically due to an autonomic nervous system disorder (dysautonomia). Midodrine is usually only prescribed when other forms of treatment have not provided adequate results. It belongs to the therapeutic group known as adrenergic and dopaminergic agents.

Caution

Pregnant women should not be using Midodrine as it might harm the unborn baby and pose a risk of miscarriage. For this reason, women planning to become pregnant should use appropriate birth control measures during the treatment. In the event of falling pregnant while being treated with this drug, consult with a doctor immediately. ProAmatine may pass into breast milk, and mothers should not feed an infant while taking the drug without first consulting with a doctor.

Dosage

Follow the prescribed dosage carefully. The recommended initial dosage is usually one tablet of 2.5 mg three times a day. This dosage can be increased weekly to reach the usual maintenance dose of 30 mg daily. The recommended daily dosage should be spread into three equal doses. The last dose should be taken at least 4 hours before bedtime. If a dose has been missed, do not take a double dose to make up for it. Stick to the schedule since taking too high a dose can cause high blood pressure when lying down.

Storage

What is the 30/20/10 rule for diagnosing orthostatic hypotension?

If doctors detect a decline of more than 20mm Hg in systolic pressure or 10mm Hg in diastolic pressure after three minutes of standing upright, they can diagnose orthostatic hypotension.

What worsens orthostatic hypotension?

Symptoms may be made worse due to behaviors such as eating large meals, dehydration, drinking alcohol, prolonged exposure to heat (such as hot baths or showers), or over-exertion.

What are the red flags of orthostatic hypotension?

Many symptoms include tachycardia upon standing, light-headedness, fatigue, exercise intolerance, and cognitive impairment. These may not be accompanied by a fall in blood pressure, and the reasons for these symptoms are unclear.
