Inspra (eplerenone) can be prescribed for patients with symptoms of heart failure who have measured a reduced ejection fraction, especially after suffering a heart attack (acute myocardial infarction). A diagnosis of heart failure with a reduced ejection fraction (HFrEF) is given when the left upper chamber of the heart is pumping out (ejecting) less than 40%. This indicates that insufficient oxygenated blood is being pumped into the body.
Inspra belongs to a group of drugs known as aldosterone antagonists. It works by blocking the action of aldosterone, a hormone that can cause sodium and fluid retention in the body.  Excessive fluid accumulation can exacerbate the conditions leading to heart failure.
Inspra reduces fluid buildup in the body and can lower blood pressure. Lowering blood pressure reduces the risk of cardiovascular (CV) events, strokes and heart attacks. Inspira also reduces the risk of severe outcomes for patients with heart failure, especially in patients who have recently suffered a myocardial infarction (heart attack).

Dosage and administration

Take the medicine as indicated on the printed label on the box. The doctor prescribes the dosage specific to your symptoms and other medications you may be taking. The tablets can be taken with or without food and should be taken at roughly the same time each day. If a scheduled dose is missed, take it as soon as possible, but if it is almost time for the next scheduled dose, skip the missed dose. Do not double-dose.

  • Breast pain or tenderness
  • Changes in libido or sexual performance
  • Dehydration
  • Edema of the lower limbs
  • Confusion, weakness, and irregular heartbeat can result from increased potassium levels in the blood.

Inspra is prescribed to reduce hypertension (high blood pressure).

The main symptoms of hypertension are:

  • Severe headaches
  • Frequent nosebleeds
  • Chest pains, especially after exercise
  • Difficulty breathing
  • Irregular heartbeat
  • Fatigue or confusion
  • Vision problems
  • Bloody urine.

High blood pressure doesn’t always have symptoms and may only show up when a doctor checks your blood pressure.

High blood pressure increases the workload on the heart and strains the arteries. If it persists without treatment, the heart and arteries may stop functioning correctly. This can lead to damage to the blood vessels of the brain, heart, and kidneys, increasing the risk of stroke, heart failure, or kidney failure. Hypertension may also increase the risk of myocardial infarction (heart attack).

  • What class of drug is eplerenone?
    Inspira (eplerenone) is in the aldosterone antagonist class of drugs. It is neither an ACE inhibitor not ARB blocker, but can be used in combination with them in treatment of heart failure with reduced ejection fraction and hypertension.
  • What drug improves survival in heart failure?
    Eplerenone at a target dose of 25 mg has been shown to improve survival in patients with heart failure and reduced ejection fraction.
  • Does eplerenone reduce mortality in heart failure?
    Eplerenone is associated with a reduction in mortality in heart failure. In one trial, the selective aldosterone blocker, eplerenone, when added to standard therapy was shown to significantly reduce all-cause mortality by 15% and the rate of cardiovascular (CV) mortality by 13% over a follow-up of 16 months.
  • How long should you take eplerenone?
    The length of treatment time for eplerenone will vary depending on the prescribing doctor's recommendation. Most people are typically treated with eplerenone for the long-term, and often for the remainder of their lives.
  • Can I eat bananas with eplerenone?
    Foods that contains a lot of potassium like bananas, avocados, potatoes, dried apricots, nuts, and salmon should be limited or avoided. Using these with eplerenone can raise potassium levels too high. It is advisable to limit consumption of such foods.
